Our Approach

Hospice care focuses on palliative or comfort care and is considered when all other medical treatments have been explored. The goal is to prevent and relieve suffering while improving the quality of life for individuals facing serious, complex illnesses at the end of life. Our services begin when your Primary Care Physician and a Hospice Physician agree that the hospice criteria for a terminal illness are present, usually with a prognosis of six months or less. Our team then conducts a specialized, individualized assessment to determine any specific care needs, tailoring a care plan that centers around personal comfort and support.

In addition to our skilled nurses, our interdisciplinary team includes experienced hospice physicians who oversee the care of patients during the final weeks or months of their lives. These physicians are dedicated to making their patients as comfortable as possible, providing direction for future care, and ensuring that each patient's unique needs are met. Our hospice physicians are flexible, responsive, and available to see patients as often as needed to alleviate suffering and provide guidance.
